Surgical Treatment of Cervical Tuberculous Lymphadenitis.



Abstract
We have reviewed 51 cases of tuberculous lymphadenitis who were operated from January 1987 to December 1988 at Seoul National University Hospital.
@ES The result are as follows:
@EN 1) There are 5 times more female patients than male and the most prevalent age group was third decade followed by fourth decade.
2) The average duration of disease was 14.2 months and 41.2% of the patients were between one to three months.
3) Unilateral location was more common than bilateral or midline and the most frequent location of the lesion was anterior cervical triangle(35.3%).
4) Signs on admission include mass, cold abscess and discharge. Palpable mass was the most common(81.3%) symptom.
5) 30 patients(58.8%) had past medical history of tuberculosis and the history of tuberculous lymphadenitis was 46.7%, followed by pulmonary tuberculosis(26.7%), unknown site(13.4%) and other sites(13.4%).
6) Patients with both positive history and negative chest lesion, negative history and negative chest lesion were detected in 35.3% of the patients, positive history and positive chest lesion was detected in 25.5% and in 3.9% there were negative
history and positive chest lesion. There was no active pulomonary tuberculosis.
7) The operative prodcedures performed was Simple Excision(70.6%). Incision and Drainage (25.5%) and Wide Excision(3.9%).
8) To whom Incision and Drainage were under went, kanamycin irrigation was performed on the six patients. Average wound duration was 56.6 days compared with Without-kanamycin irrigation group, of which the wound duration was 73.3 days. There was
no
statistical difference.
9) All patients except one received chemotherapy. The average duration of anti-tuberculous medication was 1 year and 6 months.
10) The authors experienced 7 cases of recurrences. The average interval from the initial operation was 4.3 months and of these patients four patients received Incision and Drainage, two patient Excision, one patient observation only. Two
patients
treated on admission and the others treated at OPD bases.
